1. The participant agrees to record the provider as an endorsed "My Provider" in their myNDIS participant portal or request their NDIA planner or Local Area Coordinator (LAC) to register this relationship prior to service commencement.
2. The participant acknowledges that under NDIA PACE system rules, provider claims for agency-managed or plan-managed supports may be delayed or rejected if formal portal endorsement is not maintained.
3. If an endorsement is temporarily unconfirmed on the NDIA system, the provider will notify the participant or their nominee, and both parties agree to resolve the record promptly.

2. Multi-Year Plans & Check-In Continuity

Accommodates PACE 3-year plans and periodic check-ins without requiring full contract renegotiation.

CLAUSE: PACE MULTI-YEAR PLANS AND PERIODIC CHECK-INS
1. Where the participant holds a multi-year NDIS plan managed under the PACE system, this Service Agreement remains continuously in force across plan check-in milestones unless formally terminated.
2. Any adjustments to support schedules resulting from a scheduled NDIA check-in or unscheduled plan reassessment will be documented via an updated Schedule of Supports signed by both parties.
3. Rates for supports adjust on 1 July each year in accordance with the gazetted NDIS Pricing Arrangements without requiring a full agreement reissue.

3. Unendorsed Service Delivery Contingency

Protects provider cash flow if technical portal delays prevent immediate PACE claiming.

CLAUSE: CONTINGENCY FOR PORTAL ENDORSEMENT DELAYS
1. Where services are delivered in good faith pursuant to this agreement and payment is rejected solely due to an administrative or portal endorsement delay within the NDIA PACE system, the participant or nominee agrees to cooperate in resolving the endorsement within five (5) business days.
2. In the event that portal endorsement cannot be established or is revoked contrary to this agreement, the parties agree to review funding arrangements, including self-managed or invoice payment options where permitted by law.

4. Plan Manager PACE Data Exchange

Explicit consent to share invoices and support schedules with PACE-registered plan managers.

CLAUSE: PLAN MANAGER DATA SHARING AND PACE COORDINATION
1. The participant authorises the provider to communicate directly with their nominated Plan Management provider to submit invoices, verify PACE funding categories, and confirm allocation balances.
2. The participant agrees to notify the provider within seven (7) days if they change their Plan Manager or if their plan management type transitions between NDIA-managed, plan-managed, or self-managed.
